1. Eligibility and Licensing:
   1. I am a Fijian citizen and a natural person, residing in the transport zone for which my PSV permit is issued.  
   2. I possess a valid Public Service Vehicle (PSV) driver’s license issued by the Land Transport Authority (LTA) of Fiji, and I commit to maintaining its validity throughout my engagement with Taxitics Fiji.  
   3. I have never been convicted of any serious offenses under Parts 12B, 16, or 17 of the Crimes Act 2009, or similar provisions of repealed laws or laws of other jurisdictions.  
   4. I confirm that neither I nor any member of my household currently holds or has previously held a taxi permit, unless explicitly approved by the LTA under specific circumstances.  
2. Consent for Background Checks and Data Collection:
   1. I explicitly consent to Taxitics Fiji conducting comprehensive background checks, which may include, but are not limited to, my criminal record history, professional credentials, financial indebtedness history, and verification of my personal details (full name, date of birth, residential address, Tax Identification Number (TIN), and any other names I have been known by).  
   2. I understand that these checks are essential to ensure the safety and security of passengers and to comply with Fijian regulatory requirements, including those mandated by the LTA for PSV permit holders.  
   3. I consent to the collection and processing of my personal data, including my PSV license number, contact details, and any medical reports required for my PSV permit.  
   4. I agree that Taxitics Fiji may store and process this data and, where necessary for verification and compliance, share it with relevant Fijian government agencies, including the Land Transport Authority (LTA), Fijian Competition and Consumer Commission (FCCC), and Fiji Revenue and Customs Service (FRCS). All personal data collected will be stored and processed securely, in anticipation of and compliance with the Fijian Privacy Bill and existing data protection principles under the Constitution of the Republic of Fiji (2013) and the Electronic Transactions Act 2008.  
3. Operational Compliance and Conduct:
   1. I understand that my relationship with Taxitics Fiji is that of an independent contractor, and I am responsible for my own actions and liabilities, including property damage, which is not covered by the no-fault Accident Compensation Act 2017. I acknowledge the strong recommendation to obtain comprehensive insurance for my vehicle to cover property loss.  
   2. I commit to adhering to all Fijian transport laws, including the Land Transport Act 1998 and the Land Transport (Public Service Vehicles) Regulations 2000, as well as all policies and directives issued by the LTA and FCCC.  
   3. I agree to be civil and courteous to passengers and other road users, maintain a clean and tidy personal appearance, refrain from smoking in the vehicle while on duty, and not consume alcohol or drugs while on duty or within 8 hours prior to commencing duty.  
   4. I will drive safely and carefully, obey all traffic laws, and use a mobile phone only with a hands-free device while driving.  
   5. I will ensure that my photograph, name, and PSV license number are conspicuously displayed within the vehicle at all times for passenger safety and transparency. 

Vehicle Declaration and Consent
By signing below, I, the undersigned driver, further declare and consent to the following regarding my vehicle(s):

1. Vehicle Licensing and Roadworthiness:
   • My vehicle(s) is/are legally registered as a commercial Public Service Vehicle (PSV) with the LTA and hold(s) the necessary permit(s) (LT or LH type).  
   • My vehicle(s) meet(s) all roadworthiness and national emissions standards as required by Fijian law.  
   • I will ensure that my vehicle’s taximeter (if an LT taxi) is kept in good working condition, is properly tested, and is used for all metered journeys in accordance with FCCC and LTA regulations.  
   • I will ensure that the official schedule of fares, as mandated by the FCCC, is conspicuously displayed inside the taxi where it can easily be seen by the hirer.  
   • I will provide a receipt upon request, which includes the taxi number, date, fare, VAT component, and journey details.  
2. Data Collection and Operational Use:
   • I consent to Taxitics Fiji collecting and processing information about my vehicle(s), including make, model, registration number, PSV permit type (LT or LH), and insurance details, for operational and compliance purposes.  
   • I understand that Taxitics Fiji may utilize real-time GPS tracking for operational efficiency and to ensure adherence to shortest route requirements.  

3. Acknowledgment and Agreement
   I confirm that I have read, understood, and agree to all the terms and conditions outlined in this Consent Form. I understand that my continued use of the Taxitics Fiji platform is contingent upon my ongoing compliance with these terms and all applicable Fijian laws and regulations.
